    • Job Requirements
    • American Heart Association certification is required for all direct patient care personnel.
    • American Heart Association certification is required for RNs in CCU, ED and PACU, MICU, SICU.
    • Must be dated within 30 days of start for positions requiring a license or certification.
    • 9 Panel must be completed within 30 days of start date, with MRO signature, excluding THC.
    • Must have page 27 of Trinity Code of Conduct signed and on file in Trinity VMS.
    • Standard i9 completed by the agency.
    • Trinity Health Confidentiality of Patient and Proprietary Information and Information System Security Agreement with page 5 signed on file.
    • Attach copy of specialty exam must be completed within a year.
    • Iowa Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check required for all engaged contractors within 30 days prior to start.
    • Must complete Trinity Non Employee Upload Form.
    • Must complete Trinity OFAC within 30 days of start.
    • Resume, Skills Checklist within a year, 2 Professional References required for submission.
    • All engaged contractors must have page 1 of Trinity Conflict Of Interest Disclosure Form on file in TrinityVMS.
